1. Purpose & Scope Of This Policy

This Policy sets out how Partnered Health Pty Ltd ACN 164 892 924 and its related bodies corporate[1] (Partnered Health, we, our, us) handles personal information and acknowledges Partnered Health’s obligations under the Privacy Act 1988 (Cth) (Privacy Act) and other relevant privacy laws and regulations.  
 
In undertaking our activities and functions, Partnered Health may collect, hold, use, disclose and otherwise handle your personal information.  This Policy provides an overview of our practices for the handling of your personal information in the course of our activities and functions.
 
By providing Partnered Health with your personal information, you acknowledge that Partnered Health will handle your personal information as set out in this Policy.

2. Privacy Policy

What information does Partnered Health collect about you?

We generally collect and hold your name, date of birth, postal and email address, telephone number, credit or debit card details, billing and account details, information obtained from any questionnaire which we may ask you to complete, and other information we consider necessary to provide our services to you.

Health services
 
If we are providing you with a health service, we may also collect and hold your Medicare number, individual healthcare identifier number, health fund number, veteran’s affairs number, occupational health history, medical records, medical prescriptions and other health information or sensitive information about you we consider necessary to provide our services to you or to your employer or potential employer.
Health service refers to any activity which is intended or claimed to involve assessing, recording, maintaining or improving your health, diagnosing or treating your illness or disability or dispensing prescription medicines by a pharmacist, including allied or occupational health services, or medical assessments or screenings conducts by our GP clinics.
 
Employees and contractors
 
If you are applying for a job or contract with us, we may also collect and hold your previous employment history, tax file number, next of kin, skills or competency information, documentary proof of your right to work in Australia or elsewhere, driver’s licence, bank account, superannuation details and sensitive information such as your membership in any trade or professional association.
 
Credit information
 
If we are considering providing credit to you or an entity related to you, we may also collect and hold consumer and commercial credit information about you (including without limitation credit information and credit eligibility information).
 
Sensitive information
 
Unless an exemption applies under the Privacy Act, we will not collect your sensitive information (including health information) without first obtaining your consent and ensuring that the sensitive information is reasonably necessary to conduct one or more of our functions or activities.

How does Partnered Health collect your personal information?

The below table sets out how we generally collect personal information.
Directly from youFor example, in forms and questionnaires you complete for us, and your interactions with us and our staff, such as via our websites, over the phone, via email or in person.
From our service providers, staff, therapists, doctors and contractorsFor example, from other healthcare providers, our contracted health professionals, referring doctors, consultants, clinics, IT service providers and other personnel who assist us or who provide services to you on our behalf or your employer or potential employer.  This includes from our GP clinics or medical assessment clinics.
From other people involved in assisting us with providing services to you or to your employer or potential employer or any other person who referred you to usFor example, from: your doctor and other health professionals or government sources such as Medicare, the My Health Record system, Department of Veterans’ Affairs or the electronic transfer of prescriptions (eTP) services, if we are providing a health service to you;your relatives or other people in the case of an emergency where you are physically unable to give your consent;your employer or prospective employer or any other person who referred you to us, if we are undertaking a medical assessment or screening for your employer or prospective employer or such other person;your employer or their insurer, if we are providing corporate services (such as insurance claims management or emergency travel assistance services) to you or your employer;our GP clinics, medical assessment clinics or other group entities; orcredit reporting bodies, if we are considering providing credit to you or an entity related to you.

How does Partnered Health hold your personal information?

We hold your personal information in various ways, including in paper and electronic form.  All personal information we hold is located and processed in Australia. We take reasonable steps to protect your personal information from misuse, loss, unauthorised access, modification and disclosure including through implementation of encrypted, password protected, controlled-access digital storage solutions as well as keeping paper forms in secure, restricted access filing systems.

We require that our contracted service providers who assist us in supplying services to you, and to which we disclose your personal information, have in place reasonable safeguards for protecting personal information or are subject to the Privacy Act or, if overseas, are subject to laws and regulations. We require our contracted service providers to use Australian based data centres for storage of personal information.

Why does Partnered Health collect, hold and use your personal information?

Unless you consent otherwise, or where we are otherwise permitted to do so by law, personal information will only be used for the purposes for which it was provided (primary purpose) or for a purpose related, or in the case of sensitive information directly related, to the primary purpose. Some examples of the purposes we ordinarily use personal information for are detailed in the table below.

Uses related to the provision of administration servicesTo assist with making, assessing, administering or determining any insurance claim made by you or in respect of an incident in which you were involved in.To provide travel and medical assistance services to you or to your employer.
Uses related to job or contract applicationsTo assess and determine your eligibility or suitability for a job or contract with us.To, with your consent, contact professional or other reference(s) you have provided in connection with a job application to discuss your experience/employment.
Uses related to the provision of creditTo obtain credit information related to you, and to undertake credit reference checks and otherwise assess your credit worthiness (or the credit worthiness of an entity related to you).
Use for research purposesWe may use de-identified personal information for research purposes.

How does Partnered Health use and disclose your personal information?

Disclosures related to the provision of creditTo the following credit reporting bodies (CRB): Veda Advantage Limited; Dun & Bradstreet (Australia) Pty. Limited; CreditorWatch Pty Ltd; and Experian Information Solutions, Inc.
Dealing with us anonymously

You have the option of not identifying yourself or using a pseudonym when dealing with us unless we are required by law or a court or tribunal to deal with individuals who have identified themselves or it is impractical for us to deal with you if you have not identified yourself. Generally, it is not practicable for Partnered Health to deal with individuals anonymously or pseudonymously on an ongoing basis. If we do not collect personal information about you, you may be unable to utilise our services.

Direct marketing

By providing us with your personal information, you agree that we may provide you with marketing materials in relation to offers, specials, products and services that we have available. You may opt out of receiving marketing communications from us at any time by following opt out instructions provided in such marketing communications, or by contacting us on the details listed below.

We will not collect, use or disclose your sensitive information for the purpose of direct marketing without your written consent. 

Credit information

If requested by us, a CRB may use credit information about you to assess whether you may be eligible to receive direct marketing communications from us (i.e., pre-screening).  You have the right to request that CRBs do not use credit information about you for this purpose.

Overseas disclosure

Partnered Health will not disclose your personal information to overseas recipients. In some circumstances, we may seek your consent to disclose your personal information overseas, if required for the provision of services to you.

How is your personal information handled on our websites?

Partnered Health’s websites use cookies. A “cookie” is a small file stored on your computer’s browser, which assists in managing customised settings of the website and delivering content. We collect certain information such as your device type, browser type, IP address, pages you have accessed on our websites and on third-party websites. You are not identifiable from such information. You can use the settings in your browser to control how your browser deals with cookies.

How can you access or seek correction of your personal information?

Should you wish to access your personal information held by Partnered Health, or would like to edit any information previously provided, please contact Partnered Health’s Privacy Officer using the contact details listed below.

Generally, we will provide you with access to your personal information, except in limited circumstances in which the law permits us to deny access.  No fee will be incurred for requesting access, but if your request for access is accepted, we will inform you of the fee (if any) that will be payable for providing access if you proceed with your request.

In order to protect your personal information, Partnered Health may require identification from you before releasing or amending your personal information.

It is important that your personal information is up to date and that you advise Partnered Health as soon as practicable if any of your personal information is inaccurate so that Partnered Health can update its records.

What should you do if you have a complaint about the handling of your personal information?

If at any time you have a complaint against Partnered Health in relation to this Policy, please contract Partnered Health’s Privacy Officer using the contact details below. Complaints must be in writing.  Partnered Health will deal with complaints within a reasonable period of time having regard to the nature of the compliant.
