Access to your personal and health information is necessary for us to deliver you a high standard of service and care.
What We Collect
The types of personal and health information we collect depends on the nature of your dealings with us but will generally include your name, date of birth, postal and email address, telephone number, credit or debit card details, billing and account details, information obtained from any questionnaire which we may ask you to complete, and other information we consider necessary to provide our services to you such as your Medicare number, individual healthcare identifier number, health fund number, veteran’s affairs number, health history, medical records, medical prescriptions, occupational health history and other health information or sensitive information related to the provision of healthcare services.
How We Collect Your Personal Information
Depending on the circumstances we may also collect your personal or health information from other sources, such as other healthcare providers, our contracted health professionals, referring doctors, consultants, clinics, IT service providers and other personnel who assist us or who provide services to you on our behalf.
We will use reasonable efforts to obtain consent from you before collecting your personal or health information from other sources. We will only collect personal or health information from other sources if we require it to provide healthcare services to you.
In emergency situations, or where you are physically incapable of providing consent, we may need to collect personal or health information from your relatives or other sources as required for your treatment.
How We Handle Your Personal Information
In carrying out these activities, we may disclose your personal or health information to your healthcare providers, pathology services, pharmacists, specialist doctors, WorkCover, health care facilities such as hospitals and disease monitoring agencies, government administered registers, our advisors and third parties who provide products and services to us, to our related entities and as otherwise required or permitted by law. We may disclose your personal or health information to other third parties if directed by you. Generally, we will not disclose your personal or health information to third parties located overseas except if we are permitted or required by law, or with your express consent.
Consent To Collection Of Your Personal Information
You consent to the collection and use of your personal and health information by Partnered Health as required to provide healthcare services to you, including to other doctors or healthcare providers directly or indirectly involved in your healthcare or medical treatment.
By signing this form, you consent to us contacting you with updates about Partnered Health. If you no longer wish to receive communications of this nature from us, you may opt out at any time by contacting us as set out below, or as set out in the relevant communication.
If you do not provide us with the information we request, we may be unable to respond to your enquiry or otherwise provide you with the healthcare services that you seek.
Artificial Intelligence
In some cases, your practitioner may use AI-assisted tools during your care. AI tools may be used to transcribe audio recordings of the sessions to compile session notes, or to assist in report-writing and communications. Your practitioner will seek verbal consent from you prior to using AI-assisted tools during session(s).
AI-assisted note taking software used by your practitioner complies with Australian privacy laws and Partnered Health’s internal policies regarding AI use. Personal information collected via AI-assisted tools is stored on an encrypted server and does not contribute to training AI models.
By signing this form, you consent to the use of AI-assisted tools during your care. You may opt out at any time by informing your treating practitioner.
